Scholarship Application

To be eligible for a scholarship, the following must be completed:

Complete the information below.

Provide a Letter of Recommendation from your Clergy or School Official attesting to your character.

Provide an original final report card from your school. You must have attained a minimum grade point average (GPA) based on the number of years you have received a scholarship from Baseball for Kids.
